Erlanger offers outpatient and inpatient Behavioral Health benefits. This plan is administered by Blue Cross Blue Shield of Tennessee.

Inpatient Mental Health/Substance Abuse:

• 30 days per calendar year
• In-network - $500 copay per admission
• Out-of-network - Subject to out-of-network deductible and coinsurance

Outpatient Mental Health/Substance Abuse:

• 30 visits per calendar year
• In-network - $25 copay per visit
• Out-of –network - Subject to out-of-network deductible and coinsurance

Mental Health Medication Management Benefit
Outpatient treatment visits for Medication Management does not count toward the number of mental health outpatient visits per year. Medication Management means pharmacologic management, including prescription, use, and review of medication with no more than minimal medical psychotherapy.

• Substance abuse treatment is limited to two episodes (inpatient and/or outpatient) per lifetime.
• Benefits for Inpatient and Outpatient and Substance Abuse are combined with Mental Health visits to determine yearly visit limits and lifetime maximums.
