Payment for Services

Payment is requested at the time of service and may be made by cash, check, money order, Visa, or Mastercard. Medicaid, Medicare, and most private insurances are accepted at each location. Co-payments are the patient's responsibility and due at the time of service. You may be eligible for reduced fees for services through a sliding fee discount program, but federal guidelines require patients to provide proof of income and address at the time of service. The acceptable documents to provide are as follows:

- Pay stub
- W2 forms
- State driver's license
- Utility bills

CVS Pharmacy

Conveniently located at our Dodson Avenue location, our CVS Pharmacy offers discounted prescriptions (340B pricing) to our patients.
